Saw this and thought it was neat.

Acheron produces death reports. When somebody dies, you can right-click their unit frame and click the Acheron menu entry, and you'll see everything that happened to them in the six seconds (or so; configurable) prior to their death.

So whether it's because they stood in the fire or because they really weren't getting any heals, you can find out - and link it in whatever channel you want.

GrimReaper also does this, as does Recount. But it's still a nice idea.

The "Deaths" tab is the only one I generally use on Recount. I'd recommend that if you die during a boss fight, bring up your death information and see exactly what killed you. Taking a few seconds to understand where the damage came from, and in what amounts can help you understand the boss fight better and the various damage sources. The old saying, "Those who don't learn from history are doomed to repeat it."
